Renewal of Certification
- A certified adoptive parent who has not filed a petition for adoption within eighteen months of the original certification order, may apply for an extension of certification.
- OAS will obtain, from the adoptive parent seeking renewal the following information:
- A copy of the request for renewal of certification.
- A statement of any changes in the certified adoptive parent’s social, family, medical, and financial circumstances.
- New fingerprint clearance at least every third year following original certification.
- A current medical report for all members of the applicant’s household at least every third year following original certification
- When investigating a request for renewal of certification, OAS will, at a minimum, complete the following:
- Conduct a face-to-face interview at the applicant’s home with the applicant and the applicant’s other household members over the age of five
- Investigate any change in circumstances described in the request for renewal as necessary to determine continuing fitness to adopt
- Document all action
- Upon completion of the renewal investigation, OAS will prepare and file with the Court, a report of the investigation, which will contain a recommendation for or against renewal of certification.
