Post Placement Visit and Report

1. OAS will conduct visits and write post placement reports to meet the guidelines of the country requesting the report.

2. Post placement visits will take place in the home with all family members present. The visits will be at a minimum one hour.

3. The social worker will assess the child in the following areas: physical, intellectual, emotional, and social development.

4. The social worker will report on the child’s development in the above listed areas, stating any delays or concerns.

5. The report will include how the family is providing for any special needs of the child and describe interventions the family has implemented, evaluations done, and professionals consulted.

6. The social worker will discuss with the family member their adjustment to the adoption and any concerns they may have.

7. Recommendations will be made for community services if it is appropriate.

8. The information from the home visit will be written in a report and sent to the agency or country specified by the applicant.
